Service Delivery
In Wyoming there is a Home and Community Based Waiver for Adults with Acquired Brain Injury that can serve up to 100 individuals in a fiscal year, with no more than 85 served at any one time. To be eligible individuals must be between the ages of 21-64, and must meet both clinical and financial eligibility criteria. The waiver is administered through the Division of Developmental Disabilities. A request for services is a simple one-page form that can be obtained from the Division of Developmental Disabilities or from a wide range of independent providers and other State offices such as Department of Family Services or Public Health Nurses.
Services include: case management, assessments, residential habilitation, in home support, day habilitation, pre vocational services, supported employment, cognitive retraining, personal care, respite, specialized medical equipment, environmental modifications, physical therapy, occupational therapy, speech therapy, dietician services, psychological therapy, skilled nursing, and vision therapy. A forecasting model determined an Individually Budgeted Amount for each individual. This model considers the severity of disability, personal characteristics, as well as services utilized.
There is also a small program on the campus of the Wyoming State Training School, called Visions, that provides services for up to 12 individuals with brain injury. Individuals interested in this program must first meet the eligibility criteria for the waiver although the program is supported financially by State general revenue.
Individuals with brain injury may also qualify for other waivers including the Children's DD waiver, the Adult DD waiver, and the Long Term Care Waiver.
