Service Delivery

The TBI Home and Community-Based Medicaid Waiver program is housed in and administered by the Department of Health, Office of Medicaid Management, Bureau of Long Term Care. The Division of Home and Community Based Care within the Office of Health Systems Management is responsible for surveillance of waiver providers.

Once individuals are determined to be waiver eligible, they may choose a service coordinator to oversee and coordinate their services. Services which may be provided under the waiver include: independent living skills training and development, structured day program, substance abuse treatment, intensive behavioral program, community integration counseling, home and community support services, environmental modifications, respite care, special medical equipment and supplies, and transportation. Regional Resource Development Specialists (RRDSs) are contracted with the Department of Health to oversee the provision of waiver services in their respective regions. The Department of Health's Medicaid Waiver serves individuals age 18 to 64 years old. Individuals with TBI who are injured after the age of 18 would contact the Department of Health, Bureau of Long Term Care or the DOH Regional Resource Centers for information and referral to services provided under the TBI Medicaid Home and Community-Based Waiver.

Individuals who acquired a TBI before the age of 22 may access a home and community-based waiver operated by the Office of Mental Retardation and Developmental Disabilities (OMRDD), a separate entity from the Department of Health. The OMRDD waiver includes residential habilitation, day habilitation, supported work and prevocational services. Individuals with TBI who may be eligible for these services would contact the Waiver Management Unit at OMRDD or the local OMRDD Developmental Disabilities Services Offices.

Individuals who are served by the Department of Health waiver may also be eligible for a housing subsidy program that assists in locating and paying for affordable rentals. The Neurobehavioral Resource Project assists individuals with TBI and challenging behaviors by providing: training to clinicians, staff and service providers; consultation on crisis situations; functional behavior assessments; collaboration to develop person-centered programs; advocacy for persons in the criminal justice system; apprenticeship program designed to provide direct modeling and coaching of clinical and interactive skills to address challenging behaviors.

The Medicaid Waivers, Neurobehavioral Resource Project, and rent subsidy programs have been critical to the successful repatriation of over 400 persons previously placed in out-of-State treatment facilities

HRSA-Funded Projects

The NYS Department of Health has received Implementation Grants (1997-2000) and Post-Demonstration Grants (2001-2003). The Post-Demonstration Grants have focused on improving outreach to minorities, assessing consumer satisfaction with the waiver program, and implementing best practices by enhancing training of service providers in the areas of participant rights and provider sensitivity.
