Service Delivery

Louisiana residents with TBI caused by an external physical force, regardless of age, may contact the Louisiana Rehabilitation Services State Office at the above address to request an application for services provided under the Louisiana Head and Spinal Cord Injury Trust Fund. Services include: evaluation, post-acute medical care rehabilitation, therapies, medication, attendant care, equipment necessary for activities of daily living and other goods and services deemed appropriate and necessary. Eligible consumers are assigned a Case Manager who will conduct an initial assessment and facilitate the provision of services. Services are provided on a first come basis.
