Service Delivery
In Connecticut, individuals with TBI aged 18 -64 would seek services from the Department of Social Services, Division of Social Work and Prevention Services. This division administers the Acquired Brain Injury Home and Community Based Medicaid Waiver. Applications requesting services can be secured from any DSS regional office; completed applications are returned to the Division at the DSS Central Office location. DSS social workers conduct assessments for eligibility, service needs, manage the creation of care plans, and manage the service team. Waiver participants, who must be eligible for Title XIX, can access any or all nineteen services offered through the ABI Waiver including independent living skills training, pre-vocational, supported employment, cognitive behavioral programs, companion, home-delivered meals, homemaker, personal care assistance, respite, transportation, community living support, chore services, case management, vehicle modification, environmental accessibility adaptations, etc. Individuals over age 65 or those aging off the ABI Waiver may, if slots are available, receive services through the elder ABI Pilot, administered by Eldercare in the Medicaid Division at DSS.

HRSA-Funded Projects
The Connecticut Division of Social Work and Prevention Services received the MCHB/HRSA State Planning Grant in 2003 and serves as the Lead Agency for TBI. The overarching goal is to create a coordinated and integrated system of care for individuals with TBI. As a starting point, Connecticut established a Statewide TBI Advisory Committee, is conducting a Needs and Resources Assessment of individuals with TBI and their families and assessing the resources of the State's services and supports, and is in the process of developing a TBI State Action Plan.
